Nice genuine WWII US Navy personal identification tag, issued to Edward Paul Kurkowski, service number 653-31-14, blood type A, vaccinated against tetanus in November 1942. 

Edward Paul Kurkowski was born on May 23, 1923 in Allenport, Pennsylvania in a Polish family and joined the US Navy on November 7, 1942. He went on to serve as a Machinist's Mate 3rd Class aboard the USS Reno (CL-96) from December 28, 1943 to April 7, 1945 and was discharged on July 7 of that year.

The USS Reno was commissioned in December 1943 and deployed to the Pacific, where she participated in the Philippines Campaign. On November 3, 1944, the light cruiser was badly hit by a torpedo fired by Japanese submarine I-41 in the vicinity of Luzon. Following repair, the ship was briefly back at sea in late 1945 prior to being decommissioned in early 1946.
